Physical Therapist
Status: Full-time, 80 hours per pay period

Position Purpose:
The practice of physical therapy shall include assessment, treatment, planning, implementation, education, communication and demonstration of leadership to address the needs of patients and their families, as well as to foster a positive working relations hip among professionals and ancillary staff. This person may be caring for infants, children, adolescents, adults and/or senior patients and will have the knowledge and skills to care for the patient and developmental needs of these populations.
Major Job Functions:
- Provides direct patient care, evaluates functional needs and outcomes, consults with other specialists as needed. Reassesses patient’s functional needs and adjusts care plan as indicated to ensure optimal patient outcome.
- Performs comprehensive patient evaluation, including tests of muscles, nerves, joint integrity and functional ability, including functional job analysis, functional capacity evaluation and pre-work screenings/pre-work screen development.
- Conducts therapy practice within defined standards of care and consistent with current PT Practice Act.
- Performs instruction of patient, family, staff, student or intern with consideration to physical/developmental age, ability/readiness to learn, cultural/emotional religious/social barriers. Uses diagrams and/or handouts as necessary to facilitate the learning process.
- Delivers age specific care to pediatric and geriatric clientele dependent on their growth, development and mental age/capacity. Responds appropriately to physically and mentally challenged clientele.
- Provides on-site services to business and industry including consultation with individual employees about pain and dysfunction as well as inservice/wellness/injury prevention education in accordance with applicable policies and guidelines.
- Directs the practice of support staff in order to achieve the patient’s functional goals.
- Communicates effectively with other professional and support staff in order to achieve positive patient outcomes.
- Promotes and contributes to a positive, problem-solving environment.
- Maintains established hospital and departmental policies and guidelines, objectives, quality improvement program, safety, environmental and infection control standards.

Qualifications/Skills:
- Completion of prescribed course in accordance with minimum essentials for Physical Therapist schools as prescribed by the Council on Medical Education and Hospitals of the AHA required..
- Current licensure to permit to practice as a Physical Therapist in the state of Wisconsin.

LPN/Ward Clerk – Med/Surg
Status: Part-time, 48-60 hours per pay period
Hours: Shifts may vary during week and could be 8, 10 or 12 hour shifts; every other weekend, holiday and call rotation

Position Purpose:
Under the supervision of the Registered Nurse, performs a variety of clerical duties associated with patient care unit operations. Complies with objectives, standards and policies of the department and within the parameters of his or her preparation. Cooperates with the members of the nursing and medical staffs and other hospital departments.
Major Job Functions:
- Maintains an orderly and businesslike environment at the nursing station in order to assist members of the health care team in the performance of their jobs despite high stress factors.
- Transcribes doctors’ orders per hospital policy. Prepares and maintains computer information. Completes daily order entry of the electronic medication administration record. Notifies RN immediately of all STAT orders. Updates patient Kardexes daily and with each new order.
- Stickers, assembles, completes and processes patient’s charts as indicated. Maintains an accurate clinical record, recording information accurately per hospital policy.
- Audits charts daily for incomplete or missing documentation (i.e. Progress Notes, Care Plans, Education Plans, Safety Assessment, Discharge Summaries, Medication Reconciliation, unapproved abbreviation and critical lab documentation), brings same to the attention of the appropriate member of the health care team for correction. Checks charts and removes non-permanent reports (if no notations on it) when permanent reports are obtained.
- Initiates discharge instructions in Logicare system.
- Communicates with members of the health team, patients and visitors using verbal and written means utilizing equipment such as telephone, paging system and computer. Performs message duties as necessary and will also complete requests for non-medical services for patients. Prints daily TMH emails and posts in the communication book.
- Is responsible for updating patient activity sheet for each shift. Prepares daily patient service requisitions. Sends profile and change-of-bed status to pharmacy on patient admission, discharge and transfer.
- Initiates daily worksheets, such as Intake & Output, daily flow sheets, charge sheets and is responsible for obtaining lab and x-ray reports and notifying RN of results.
- Prepares special forms such as body-release permit. Obtains closed records from HIS as needed.
- Orders unit supplies to maintain inventory of the unit’s equipment and stock and supplies. Prepares requisitions and reports for maintenance repairs.
- Completes patient call backs on a daily basis.
- Shows dependability by being punctual; maintains consistent attendance.
- Attends staff meetings at least 50% of the time. Reads and initials all meeting minutes of those not in attendance.
- Maintains a good working relationship among all nursing unit personnel and other hospital employees through appropriate communication. Willing to identify problems and participate in problem solving process. Demonstrates the ability to evaluate suggestions and criticism objectively.

Position Purpose:
Under supervision of the registered nurse, provides direct nursing care for infant through geriatric patients on the medical/surgical units by instituting the minimum standard of care (MSOC).
Major Job Functions:
- Provides for patient’s personal hygiene.
- Gives bedpans, urinals, baths, backrubs, shampoos, shaves and assists with showers.
- Provides for activities of daily living.
- Assists with meals and feeds patients.
- Ambulates, turns and positions patients.
- Provides fresh water and nourishment between meals.
- Administers enemas, douches, nonsterile dressings, surgical preps, ice packs, heat treatments and sitz and therapeutic baths.
- Performs I&O.
- Checks vital signs and weight.
- Performs sugar and acetone urine testing, specimen collection and post-mortem care.
- Provides for comfort needs of patients.
- Works effectively as a team member in the delivery of care and assists in the orientation of new employees.
- Utilizes resources and materials in an efficient and safe manner.
- Maintains a safe and attractive environment for patients and co-workers; transports patients and equipment as appropriate.

Status: Full-time; 72 hours/pay period
Hours: Shifts may be 12, 10 or 8 hours; weekend and holiday rotatation
Reports To: ER Medical Director/ER Director
Position Purpose:
Works collaboratively with physician in ER and under the supervision of the Medical Director of the ER to provide diagnostic and therapeutic health care to patients.
Major Job Functions:
- Performs emergency and urgent care examinations, determines diagnosis and develops treatment plan.
- Refers to ER physician as needed.
- Writes prescriptions and performs office procedures within the scope of expertise, protocols and available equipment.
- Determines and advises patients on treatments of medical conditions; provides health care information, counseling and guidance.
- All other duties as assigned.
Qualifications/Skills:
- Successful completion of an approved Nurse Practitioner or Physician Assistant program
- Valid WI license as a RN or PA
- Current State of WI and National Certification NP or PA required
- ER experience required
- CPR & ACLS certification required

Cook/Tray Aide
Status: Limited part-time position, hours as needed by department

Reports To: Nutrition Services Director
Position Purpose:
As a Cook: The Cook prepares meats, vegetables, soups, sauces, etc., for the regular and modified patient service and cafeteria service, as determined by daily production sheets. The Cook works under the direction and supervision of the Nutrition Services Director and the guidance of the Dietician. As a Tray Aide: The Tray Aide is responsible for following patient menus accurately on the tray line and for the proper cleaning and sanitation of soiled dishes, pots and pans. The Tray Aide is also responsible for serving meals in the cafeteria.
Major Job Functions:
- Puts up stock using proper storage procedures, e.g., similar items together, labels facing front, dating when necessary, etc.
- Keeps work area clean and sanitizes work surfaces; mops floors according to procedure; performs assigned cleaning duties. Sanitizes all equipment and workstation after each use.
- Follows cafeteria and/or production sheets demonstrating accuracy in weighing, measuring and portioning food.
- Knows and follows hospital and department policies and procedures, including safety standards and fire and disaster plans.
- Assumes other departmental positions or duties, e.g. washing dishes and/or assisting with special functions as requested by a supervisor.
- Works cooperatively with co-workers in evaluating cafeteria lunch leftovers and plans for use; labels, dates and stores leftovers properly.
- Consults production sheets for thawing and doing advance preparation when possible; completes and posts production sheets daily.
- Operates electrical and steam cooking equipment and dishwashers; cleans equipment and pots and pans after use; initiates repairs as needed.
- Prepares high quality food according to standardized recipes in the correct amounts in a cost-conscious manner for patients and cafeteria.
- Assists with the development and standardization of recipes, including new ways to present food attractively and nutritiously.
- Takes temperatures of hot foods prior to mealtimes and records such on production sheets.
- Sets up cafeteria hot foods at lunch and provides cafeteria backup at lunch with cold and hot foods, supplies and assistance as needed.
- Carries out the master menu plan, adjusting for nutritionally equivalent substitutes, leftovers and emergencies; records hot and cold food menu changes on production sheets.
- Plans cooking schedules to have food prepared on time, but not too far in advance; serves hot food attractively and efficiently.
- Demonstrates understanding and working knowledge of proper techniques for food preparation, including modified diets.
- Implements diet changes, prepares late supper trays and records it on the production sheet.
- Prepares and portions food for the elderly nutrition program.
- Prepares salads, coffee and desserts in accordance to meal schedules.
- Prepares late trays as required.
- Prepares and delivers juice and patient snacks to nursing units as required.
- Takes tray carts to the floor and assists in distribution of the trays to the patients.
- Delivers food carts to designated places and ensures that carts are returned to the kitchen.
- Cleans work areas, equipment, walls, cabinets, storage areas and other areas assigned in accordance with established procedures.
- Washes dishes, cookware, flatware and trays using approved procedures and properly stores them after they have been washed.
- Follows patient menus accurately in the tray makeup line.
- Answers the telephone and delivers items to units.
- Unloads groceries and places them in the appropriate locations. Maintains storeroom, walk-in cooler, equipment and workstation in clean and sanitized manner.
- Picks up cardex from nurses’ station at 5:30am and checks tray cards with cardex and makes necessary changes. Takes count of all diets and records them and gives the count to the cook.
- Sanitizes vending machine according to schedule.
- Prepares between meal supplements and records charges for all supplements for patient billing.
- Assists with preparing food for the vending machine and fills as needed.
- Assists with preparation for special functions as needed.
- Labels, dates and stores food items in appropriate storage areas.
- Performs other duties as assigned.
Qualifications/Skills:
- High School diploma or GED equivalent required.
- Cooking experience preferred.

Medical Technologist, MT ASCP
Status: Part-time, 64 hours/pay period
Hours: Primarily 3:00pm – 11:00pm and 12 noon – 8:30pm, may also work other shifts as
needed, weekend, holiday and call rotation

Reports To: Laboratory Director
Position Purpose:
Under the direct supervision of the Laboratory Director, performs clinical laboratory procedures while applying scientific principles and knowledge of physiological conditions effecting test results in order to assure high quality laboratory services. This position also performs quality assurance and instrument maintenance. Medical Technologists also maintain cooperative working relationships with intradepartmental and interdepartmental personnel.
Major Job Functions:
- Performs routine and special laboratory tests within designated area.
- Recognizes test inconsistencies and takes appropriate corrective action. Provides assistance to laboratory co-workers in resolution of similar problems. Seeks help from the Director or clinical pathologist.
- Calls critical test results to physicians or nurses immediately and according to established criteria and documents such calls.
- Prepares test reports legibly. Initials and dates work performed as needed. Reviews reports for clerical accuracy and clinical indications.
- Performs quality control tests within a designated area.
- Runs quality control tests according to procedure and maintains appropriate data recording and review process.
- Analyzes quality control data and takes appropriate corrective action when results are outside acceptable limits. Provides assistance to laboratory co-workers in resolution of similar problems. Seeks help from the department Director as required. Documents actions taken.
- Holds patient results until out-of-control situations are resolved. Reports unresolved situations to the section head, or when appropriate, to the clinical pathologist or Medical Director.
- Performs preventive maintenance and troubleshooting.
- Observes and follows departmental preventive maintenance schedules.
- Documents all instrument checks, preventive maintenance, routines, problems and corrective actions taken.
- Recognizes, analyzes and takes corrective action to resolve instrument/clinical problems. Provides assistance to laboratory co-workers in the resolution of similar problems. Seeks help form the Director as required.
- Reports unresolved instrument/clinical problems to the Director.
- Provides in-service training to other laboratory personnel and students within a designated area.
- Provides instruction in laboratory bench procedures, instrument/clinical troubleshooting techniques, and clinical application of test results.
- Assures assimilation of procedures and evaluates troubleshooting techniques. Documents same by initialing department checklist.
- Assists the Director in establishing educational programs that will meet the needs of employees or students.
- Writes new procedures and method verifications as requested under the guidance of the Director.
- Performs clerical functions associated with the designated laboratory area.
- Performs data entry of specimens accurately.
- Assures accurate billing of all tests. Corrects and makes changes as needed.
- Contributes to the general laboratory functions and institutional needs.
- Performs specimen collection and processing duties as needed or assigned and according to policies established in the phlebotomy area.
- Attends regular departmental staff meetings and in-service training and keeps current on Lab communication.
- Attends and participates in the preparation and/or presentation of the laboratory continuing education programs.
- Observes laboratory and hospital policies, procedures and safety practices.
- Keeps work area neat and clean. Restocks daily supplies and orders or notifies appropriate staff as needed.
- Demonstrates competence in performing venipunctures and arterial blood gases on all relevant age groups and in obtaining neonatal heel sticks and finger capillary blood specimens.
- Performs drug collections (DOT and non-DOT), breathalyzer testing and legal blood draws as assigned and according to policy

Qualifications/Skills:
- Bachelor degree in Medical Technology, Clinical Laboratory Science or chemical, physical or biological sciences is required.
- ASCP certification required.
