Tomah Memorial is proud to offer Hospice and Palliative Care in our new 10,000 sq. ft, Serenity House and office complex at 601 Straw St., on Tomah’s north side.
Opened in October 2011, the facility –located across from Tomah’s LaGrange Elementary School–includes rooms to care for up to 8 Hospice patients with state of the art equipment.
Each private room is individually decorated just like home, complete with bathroom and shower facilities, flat screen television, sitting area and walk in closet.
|Individual patient rooms offer a home-like setting, complete with state of the art medical equipment.|
All rooms also provide state of the art medical features, including overhead patient lifts for care and safety, computerized record keeping, along with 24-hour/day care by licensed practical nurses and a certified nursing assistant who will help with personal cares, medications and meals.
A separate kitchen, living area, library room and outdoor patio enhance the home setting.
The facility also includes clinic space for our Hospice Palliative Care program known as Life Choices, which offers specialized medical care for people with serious illnesses.
More than 80 Palliative Care patients currently receive relief from symptoms, pain, and stress of the illness thereby improving quality of life for both the patient and family members.
Tomah Memorial has been providing Hospice Touch and Palliative Care Life Choices services to more than 200 people in five counties in western Wisconsin each year. Besides the Tomah Serenity House and Hospice/ Palliative Care offices, we offer outreach offices in Mauston and Adams Friendship.

Questions Regarding Payment
The room charges for Serenity House are separate from the hospice benefit provided by Medicare, Medical Assistance and insurance plans. The hospice social worker can help you determine if your insurance plan covers the hospice residence. The daily charge is negotiable with a sliding fee scale. Application for a reduced rate can be make by completing a Tomah Memorial Hospital community care form. Memorials and other donations to Hospice Touch are welcome, and they help make up the difference for those who cannot pay the full fee.
