Hospice / Palliative Care

Hospice Touch is a program of Tomah Memorial Hospital in Tomah, Wisconsin that helps dying people be comfortable and also helps their families and caregivers so the person can stay at home. 

The new Tomah Memorial Hospice Serenity House, and Hospice / Palliative Care Clinic is located at 601 Straw St., in Tomah.

Life Choices Palliative Care is a program of Tomah Memorial Hospital in Tomah, Wisconsin for people who have a serious illness that makes it hard for them to live life as they would like. They may still be trying to cure the illness and want help managing and dealing with it.

Both Hospice Touch and Life Choices Palliative Care are programs of Tomah Memorial Hospital. Care is usually done in the persons home by a trained Team that includes nurses, nurse practitioners, chaplains, volunteers, social workers, and doctors. Both programs deal with comfort, help for the caregivers, and each persons right to make informed choices about their care and goals.

The main office of Hospice Touch and Life Choices Palliative Care is at 601 Straw Street in Tomah and satellite offices are located at Mile Bluff Medical Center, 1050 Division St. in Mauston, Wisconsin and in the CWCAC Building, 1874 Hwy. 13, Friendship, Wisconsin.

Our hospice residence called the Serenity House is located in Tomah, Wisconsin and is for hospice patients who cannot stay in their own home.

Both Hospice and Palliative Care can provide in home care within a 30-40 mile radius from Tomah and Mauston which includes parts of Monroe, Jackson, Juneau, Adams, Sauk and Vernon counties.
