Swing Bed / Respite Programs
This skilled care could only be provided in a nursing home in our community prior to 1985 and was not reimbursed by federal funds (Medicare). Since 1985, Tomah Memorial Hospital has been approved and licensed to provide this skilled care. Under the Swing Bed Program, Medicare agrees to reimburse Tomah Memorial Hospital for skilled care provided if your diagnosis and treatment plan meet Medicare's strict standards. Daily skilled nursing or rehabilitation services must be provided. Custodial care is not covered.
As a Swing Bed patient, you are expected to be gradually responsible for daily tasks such as hygiene, bathing, dressing and to actively participate in your rehabilitation through motivated performance of exercises and activities, which further your goals of returning home. If you are having difficulty in any aspect of these daily tasks, we will help you become more independent through strengthening exercises and adaptive equipment, if required.
When a patient's condition improves to the point where skilled care criteria is not met, the patient, together with our multidisciplinary staff will work toward a plan for continued care. This could include Home Health Care, Home Delivered Meals, Social Services, Personal Care Aide, Home Physical Therapy or Occupational Therapy, or placement in an Extended Care Facility and/or help from family.
It is our goal to provide care to those patients from the acute phase of an illness until he/she is ready for discharge home or an appropriate transfer.

Respite Care is designed to let you, the caregiver; take a short break from the continuous care of homebound individuals who require supervised nursing care. Our program allows you to take time off knowing your loved one is being cared for by our competent staff.
Respite Care provides a temporary living environment for individuals needing short term, supervised nursing care for one to seven days, depending on your particular circumstances. Respite Care is not designed to be a substitute for nursing home care.
To ensure its success, it is important that you have realistic expectations. Respite Care is not for everyone. Respite Care is not intended to be a substitute for long term nursing home care or long term home health care. For different reasons, our program may not prove suitable for your needs, and other care programs may be more appropriate.
